By default the e-mail notifying approval change status looks like as follow:

By default the email recipients include approvers and the assignee. 

Depending by the setting applied on the Recipients Administration screen the e-mail recipients can differ.

Depending by the setting applied on the E-Mail Templates Administration screen the e-mail content can differ.
